DISABILITY POLICY ANALYSIS |

Researchers at the Beach Center on Disability wish to invite you
to participate in an analysis of disability law using the Center's
computer assisted core concepts tool. The Beach Center on Disability
at the University of Kansas supports the protection of human subjects
participating in research. The following information is provided for
you to decide whether you wish to participate. |

What Will You Do in the Project? |
| |
There is one activity in which you are asked to participate, an
analysis. To conduct the analysis, you will register and log onto
a website from an internet-capable computer. Provisions of the chosen
statutes related to aspects of each of the Center's "core concepts
of disability policy" will then be presented to you alongside
"model language." Comparison of the provisions with the
model language will provide the basis for a score that you will assign
to the statute for each aspect. You will also be given the opportunity
to comment on each aspect. The analysis may last anywhere from 30
minutes to 2 hours. The researchers will study and organize your scores
and comments, and those of others who participate, to compile written
reports. The written reports based on the scores and comments will
be used to improve policies, practices, and educational initiatives
that respond to the needs of individuals with disabilities and their
families. If you agree to participate in the study and change your
mind later, you may drop out at any time without penalty. |

Risks of Participation |
| |
You will be asked to register before using the analysis program
and personal information may be disclosed. There is also the risk
that someone could associate you with your score of a statute if the
information is not kept confidential. |

Protections to Address Risks |
| |
You are under no obligation and may decline to answer any question
you do not wish to address, or even end the analysis, at any time,
without penalty. |

Your personal information will not be given, sold, or otherwise
distributed to anyone outside of Beach Center Staff. All personally
identifiable information will be kept in a secure place and no one
outside of Beach Center Staff will have access to such information.
Senior researchers at the Center will review and authorize on a case
by case basis all access to personal information by non-senior research
staff. |

If you decide that you do not want your information used, your information
will be kept totally confidential and will not be a part of any written
reports. We will address any concerns or suggestions that you have
about how information you share with us is used. |

We assure you that your name will not be associated in any way with
the research findings. |

You may find that using the analysis tool will increase your understanding
of the statute. |

You may find that using the analysis tool will increase your understanding
of some of the most important concepts in disability policy creation
and implementation. |
